When cardiomyocytes are injured and necrotic, cTn (cardiac troponin) is released into the peripheral blood through the damaged cell membrane. Cardiac troponin is a complex formed by three subunits of troponin C, troponin I and troponin T. In the blood of AMI patients, cTnT mainly exists in free form, while cTnI and cTnC exist in the form of complexes. The detection level of early cTnI and cTnT detection systems is ng/ml, while the modern high-sensitivity detection system can reach the level of pg/ml, and potential AMI patients can be found in the first 1-3 hours. This development has improved the level of diagnosis and risk stratification of ACS, and also made troponin (Tn) a recognized indicator with high sensitivity and high specificity in clinical practice and is recommended by all guidelines.
